We aim to treat each patient with compassion, dignity and a commitment to listening to each patient's concerns so that we can aggressively treat our patients ailments. With that focus, the most common services and insurances accepted are listed for your convenience, but this list is not comprehensive and subject to changes. Please contact your insurance company to determine if we are a participating provider in your network. We will submit insurance claims to your insurance provider as a courtesy but cannot assist you in understanding your insurance benefits. If you have questions about your insurance benefits, please contact your insurance company directly. We offer a discount for self pay and uninsured patients. We request all balances on accounts be paid in full and are unable to arrange payment plans. If our attempts to help accommodate your financial obligations to us fail, we will forward your account charges to a collection agency.

MEDICATION REFILL INFORMATION
Please always speak to your pharmacist about refills prior to calling the office. If you have run out of refills you will need to schedule an appointment as this is an indication you are overdue for a follow up appointment.
ANNUAL PHYSICAL INFORMATION
Annual physicals or preventative visits address your overall physical and emotional well being by evaluation of your lifestyle behaviors, screening labs, as well as current medications and vaccinations and review of chronic but well-controlled medical conditions like blood pressure, arthritis, diabetes, etc with the aim of developing a long term goal. Regular office visits differ from preventative care in that either new-onset or other chronic medical conditions which need further evaluation are addressed on an ongoing basis to work towards a specific goal. These types of problems need to be addressed in an appointment separate from a preventative exam. If, however, your preventative services have been adequately covered during your scheduled physical, and if there is extra time available, you may be able to address other concerns with your provider. With this is in perspective, insurance carriers do allow providers to address additional complaints beyond your physical examination if there is time to do so. However, a regular office evaluation code will be added to the preventive visit code which will entail an additional copay, coinsurance, or deductible. Also depending on the degree of difficulty or the amount of time spent, a regular office fee in addition to your preventative visit may be charged. Your provider will speak with you about this prior to the same. Please feel to talk to the staff if you have any questions regarding your preventative care charges today.
